I like to have forms that I just circle instead of fill in to make it go quicker. Here is my share of an adapted check in sheet. There are two files - margins on left and right - each one has one week. They are made to be printed/copied back to back so you can keep two weeks of data on one form (like my share of an adapted conferring sheet already posted) and put it in a binder.
